Texas Health Resources is pleased to offer you member information via a Web-based version of our computer systems called MyChart.

Patients who wish to participate will be issued a MyChart activation code during a visit to a Texas Health Hospital or Texas Health Physician Group clinic. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you can sign up online via the Texas Health MyChart App or MyChart Website.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Response to Electronic Communication

Texas Health Resources will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available, so a patient should allow at least three (3) business days for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Web Site Links

MyChart may offer links to related medical Web sites not managed by Texas Health Resources. These Web site link(s) are for a patient’s informational purposes only. Texas Health Resources does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

Minor Eligibility

An individual must be age 15 or older to request a personal account. Parents, guardians or other parental proxies of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. We cannot offer the full spectrum of MyChart services to proxies of minor patients. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 15 or older may not view without consent of the minor patient. Because of these requirements, we cannot offer the full spectrum of services to parents of minor patients in this age category. Thus, when a minor patient reaches age 15, restrictions will be placed on MyChart access until he/she reaches age 18.

Proxy Access

Individuals age 15 or older may grant proxy access to their own MyChart account by sending a proxy invite from the patient's MyChart account or completing the Proxy Access Form and either returning it to the physician where they have a MyChart account or submitting it to the Texas Health - Health Information Management department (same address listed below). Such access should only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information, or if the patient provides written authorization. If eligible, a MyChart account must be activated for both the proxy and the account holder. A proxy's access can be revoked anytime within the patient's MyChart account or by informing their physician’s office for which they have a MyChart account or by sending written notice to Texas Health Resources, Attention Health Information Management-MyChart Proxy 500 E. Border Street, Suite 700, Arlington, Texas 76010. Individuals age 15 or older may request proxy access to another individual’s MyChart account by completing the Proxy Access Form and either returning it to the physician where they have a MyChart account or submitting it to the Texas Health - Health Information Management department (same address listed above). Texas Health Resources reserves the right to revoke proxy access at any time for any reason.

E-mail Privacy

Patients who are users of MyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to a patient’s e-mail will be able to see this notification. This could include the patient’s spouse, employer or anyone else that can access a patient’s e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.

Please know that if you send us an e-mail communication, it may be shared with the Texas Health Resources staff that assists the physician in providing the patient’s medical care. A patient’s confidential medical information on MyChart will be accessible only to appropriate clinical staff.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Texas Health Resources in any other medium. Texas Health Resources is committed to protecting the confidentiality of your medical information. We limit Texas Health Resources employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyChart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart must be accessed using a secure browser. The latest versions of Internet Explorer, Firefox or Chrome are recommended. Along with user authentication, MyChart employs industry standard encryption protocols to protect your information. To verify you are on a secure connection, the address bar will begin with https://. This means your data cannot be read or deciphered by unauthorized individuals.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Texas Health Resources by viewing the digital certificate. This certificate verifies the connection between the Texas Health Resources server's public key and the server's identification.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Sharing your health information with 3rd party applications

Texas Health gives you the ability to share your health information with 3rd party applications of your choosing. To enable sharing, you must have an active Texas Health MyChart account. You will use your Texas Health MyChart username and password to authenticate with the 3rd party application.

To share your information, establish an account with the 3rd party application and review the application’s Terms & Conditions. Follow the instructions to find and select Texas Health. At the Texas Health MyChart log-in screen, enter your Texas Health MyChart username and password. Review the information that will be shared with the application which may include allergies, conditions, medications, lab tests, family history, goals, immunizations, procedures, smoking status, vital signs, and documents. If you consent, click Allow Access and continue to follow the steps provided by the 3rd party application.

To stop sharing your health information with a 3rd party application, log into your Texas Health MyChart account and go to Settings and click on Linked Apps and Devices. In the Services Accessing My Account section, find the application you want to stop releasing and click the Remove Access button.

Texas Health does not endorse 3rd party applications and is not responsible for testing applications. If you encounter a problem with a 3rd party application, please contact the application developer. Texas Health reserves the right to disable 3rd party applications if needed.

SMS/Text Acknowledgment

You acknowledge and represent to Texas Health that you are the current wireless service plan subscriber and/or an authorized user of the mobile phone number(s) that you link to (sign up to receive) the text alert/messages service, or you have been granted permission by the wireless service plan subscriber and/or authorized user of the mobile phone number(s) to enroll the mobile phone number(s) in the service;

You grant Texas Health express permission to send automated text messages to the enrolled mobile phone number(s) through your wireless phone carrier unless and until such permission is revoked in accordance with these Terms and Conditions; and

By granting such permission to Texas Health, you are hereby requesting to receive text messages in spite of the fact that your mobile phone number(s) may otherwise be on the federal, or a state's, Do Not Call List and you agree that, to the maximum extent permitted by law, your request overrides any prior Do Not Call request for or related to the Texas Health program you enroll in. You are not required to agree to receipt of text messages in order to purchase any product or service from Texas Health. Depending on the Texas Health program you enroll in, you may receive marketing messages. Texas Health is committed to building user trust and confidence by promoting and complying with the use of business practices that help protect the privacy and the security of the customer and their data.

*Terminology: "Alerts", "text Alerts" or "text messages" may be used interchangeably in any other published information about this Texas Health service. All terms referring to this messaging service are considered bound by these Terms and Conditions.

Short Codes: The Texas Health short codes are 84658 and 894584

Texas Health offers one-time text message programs as well as subscription text message services on short codes 84658 and 894584. These text messages may be operational alerts/reminders or educational health content or may contain marketing content based on the communication preferences and authorization provided by the member.

Texas Health offers these text message programs to provide information or resources on topics including but not limited to the following: Operational alerts/reminders such as billing/payment, claims, coverage, digital tools and communication materials, health and wellness tips, medical coverage, medication reminders and health insurance education. Health-related information provided is not meant to replace professional medical advice. Operational information provided is valid only at the time the text message is sent and may change at a later time.

Replying STOP from your mobile phone.

With either of these methods, you will receive one final confirmation text message from Texas Health saying that you will no longer receive text messages from that short code. If you change your preferences through Texas Health MyChart as described above, it may take up to 72 hours for your changed preferences to take effect. During this 72-hour period, you may receive additional text messages from Texas Health. You agree that your prior express consent to receive text messages shall continue indefinitely unless and until you cancel your prior express consent.

If revoking your consent by texting STOP to the text message you have received, your text response should contain only the word STOP without any additional words, spaces or characters either before or after the word STOP. This will opt you out of the text message type to which you have responded.

Texas Health will reasonably endeavor to comply with other text communications sent in which you indicate a clear and unmistakable intent to revoke your prior permission; however, you (a) understand that these messages go to a short code number that is entirely automated (is not monitored by a human) and the ability of our short code box to interpret your intent is severely restricted and, therefore, (b) you understand that unless you reply only with the word STOP, your intent to revoke your prior express permission may not be effective.

Furthermore, by agreeing to receive text messages, you hereby grant Texas Health express permission (without qualification) to respond to your cancellation request with a confirmation of cancellation text message that will confirm receipt of your cancellation request as well as indicate that you will no longer receive text messages from Texas Health. Furthermore, you agree that Texas Health will reasonably attempt to send you the confirmation of cancellation text message in a timely manner.

Texas Health will use automated technology to deliver text messages to you. By your activation of text messages, you give your consent to Texas Health to use such equipment to deliver automated text messages to the telephone or cell phone number(s) to which you have directed Texas Health to send these text messages.

Get Help or Support: To get help or answers to your questions, call 855-487-3926. From your mobile phone, you can request that we send you an additional text message that will include relevant contact information by replying HELP to the text message you received.

Pricing: Message and data rates may apply. Texas Health does not charge a fee for text messages, but mobile phone carrier charges may apply. Check with your wireless phone carrier for your plan details. By providing your consent to receive text messages from Texas Health, you approve any such charges from your mobile carrier. Charges for text messages or data usage may appear on your mobile phone bill or be deducted from your prepaid balance by your mobile carrier.

Message frequency: Number of messages received varies per user. If you have opted in to receiving health, educational or marketing engagement messages via text, you expressly consent to receive text messages on these topics no more frequently than three times per week.

If you have opted into Texas Health text messages, you expressly consent and acknowledge that some messages are sent based on your interaction with your health plan or the website and will vary depending on your activity.
